Regarding the objectives, the main one is to analyze the effect of the value chain in the competitiveness of the Portoviejo private clinics, identifying the competitive advantages to know their positioning in the local market and to determine the level of the client satisfaction with the functional and organizational procedures of the investigated private clinics. To finish, a value chain system will be designed for the private clinics. To gather the data and meaningful information, many visits will be made to the clinics in order to know their inner and outer commercial reality. Next, countless sessions of interviews, surveys will be carried out as well as observations of the current situation of the three clinics. In essence, value change is an evaluation tool to carry out an internal analysis of a company through the disintegration of its main activities. Besides, the value chain adds value to the products or services. Finally, thanks to an efficient value change, the companies give to the consumers what they want. In the administration that uses a value chain, three goals are hoped to achieve: To provide a quicker and more efficient service than that of the competition. When measuring many activities of that service, new ways to save money can be obtained. A good service is created along with the protection of the environment and nature. In this thesis, by means of an investigation made by two investigators of the UTM, it is confirmed that, in Portoviejo, the previously mentioned private clinics possess and use a value chain to satisfy the medical necessities of their clients. With regard to the conclusions, the value chain is not carried out in its entirety in the clinics of Portoviejo. Anyway, thanks to the professionalism, kindness and hospitality, the work of the private clinics is carried out in an atmosphere of cordiality and respect. In conclusion, the clinic San Antonio, the oldest in the city of Portoviejo, with 35 years of uninterrupted service, maintains a magnificent system of service to the client that makes very pleasant the visit to their facilities. Lastly, every time that a commercial business is created legally and respecting all the standards of quality and service, the city gets rich. Without any doubt, in the surroundings of the three studied clinics, the commercial value of Portoviejo city rose and it continues improving. Also, we conclude that the use of the value chain enhances the operation of the administrative and commercial departments as well as the health services of a private clinic. In the case of the recommendations, it is advised that the theory of the value chain of Michael Porter could bring many benefits to the Portoviejo private clinics. Also, all the private clinics should create a database on each patient's clinical history and this information should be accessible for the medical staff, after obtaining the due authorization. With this, the violation of the patient's privacy is impeded. In order to arm a true chain of value, the administrators of the private clinics should make sure that all the links or departments are joined together in such a way that the user tries commercially only with a person.
