ERRORES PRE-ANALÍTICOS Y SEGURIDAD DEL PACIENTE EN LOS LABORATORIOS CLÍNICOS DEL HOSPITAL GENERAL DE CHONE

Abstract

The pre-analytical phase is an important component of the laboratory where a large number of professionals intervene and cover all the steps that mediate from the moment the analysis is requested until the sample is processed. The study of errors produced in this phase is currently a very important part of the quality management systems, framed within the culture of patient safety. In this work, the rejection matrices of the clinical laboratory of the General Hospital of Chone of the year 2018 were analyzed and the personnel working in the laboratory were surveyed, with the aim of evaluating the pre-analytical errors in the clinical laboratories of said hospital. 129 pre-analytical errors (relative frequency 0.33%) were identified, most associated with the extraction process, coagulated sample (72%), inadequate volume (18.6%), hemolyzed (2.3%). 70% of the errors were reported in the hematology section, 40% of the samples came from the emergency service and 66% were concentrated in the day work shift. The deficiencies raised by the clinical laboratory staff in the surveys were in correspondence with the information reported in the laboratory rejection matrices. Since the origin of most of the reported errors is the sampling, it is crucial to train staff on how to correctly perform phlebotomy and its importance in patient safety
