Abstract
Estas tablas se han acoplado a las características de la población norteamericana mediante un proceso de calibración bien valorado y se han validado en población norteamericana . Permiten estimar el riesgo de un acontecimiento coronario (angina, infarto de miocardio con o sin síntomas, mortal o no) a 10 años. Para estimar el riesgo debe seleccionarse la tabla correspondiente a la presencia o ausencia de diabetes, el consumo de tabaco, el sexo y la edad del/la paciente. A continuación debe buscarse la intersección de su presión arterial sistólica y diastólica con su colesterol total buscando la columna de valor central ± 20 mg/dL (± 0,5 mmol/L) que incluya el valor deseado. El valor dado en la casilla hallada por este procedimiento indica el riesgo a 10 años, y el fondo pertenece al código de colores cuya leyenda se encuentra al pie de las tablas. Si se dispone del valor del colesterol de HDL puede corregirse el riesgo hallado multiplicándolo por 1,5 si está por debajo de 35 y por 0,5 si está por encima de 59. La lectura es directa si el valor se encuentra entre 35 y 59 mg/dL. 1: Los usuarios con valores extremos de presión arterial (por ejemplo, sistólica >180 mmHg o diastólica >110 mmHg) y de colesterol no precisan de cálculo de riesgo para instaurar tratamiento farmacológico. En los usuarios con hipercolesterolemia familiar, hipertrigliceridemia o con historia familiar de muerte cardiovascular prematura el riesgo es superior al indicado en las tablas. 2: Los datos de los estudios de validación indican que REGICOR es la más válida de las funciones de riesgo disponibles en ee.uu para población de 35 a 74 años. Predice adecuadamente la tasa real de acontecimientos coronarios a 5 años en población de toda norteamerica . 3: Se recomienda utilizar los siguientes puntos de corte para tomar decisiones terapéuticas sobre tratamiento de la dislipemia [5,7]: < 5% Riesgo bajo 5 - 9,9% Riesgo moderado 10 - 14,9% Riesgo alto ≥ 15% Riesgo muy alto
